Restorative Care Pathway
After hospital, illness or decline, make progress again.
Short-Term Restorative Care now sits under the Restorative Care Pathway in Support at Home. It gives older Australians short-term allied health and nursing support after a fall, hospital stay, illness, injury or decline in how they are managing day to day.
This is not just coordination after a decline. It is goal-based clinical support.
The pathway is built around a goal plan, an individualised budget, and a multidisciplinary team that may include physiotherapy, occupational therapy, exercise physiology and nursing. Avanza Care is backed by Avanti Health’s allied-health clinicians, so restorative goals and ongoing Support at Home planning can be considered together where appropriate.
