Restorative Care Pathway

After hospital, illness or decline, make progress again.

Short-Term Restorative Care now sits under the Restorative Care Pathway in Support at Home. It gives older Australians short-term allied health and nursing support after a fall, hospital stay, illness, injury or decline in how they are managing day to day.

Why provider choice matters

This is not just coordination after a decline. It is goal-based clinical support.

The pathway is built around a goal plan, an individualised budget, and a multidisciplinary team that may include physiotherapy, occupational therapy, exercise physiology and nursing. Avanza Care is backed by Avanti Health’s allied-health clinicians, so restorative goals and ongoing Support at Home planning can be considered together where appropriate.

Commonwealth Home Support Programme

Entry-level help at home — with the next pathway kept clear.

CHSP is entry-level, government-subsidised support for older Australians who need some help to stay independent and safe at home. It is often the right first step after a My Aged Care assessment, with Support at Home becoming relevant when needs are more ongoing or complex.

Why the first call matters

CHSP is simple in theory. Your referral can still be hard to use.

After your assessment, you may be approved for specific CHSP services. Avanza Care can help you understand what you have been approved for, whether our local team can support those entry-level services, and when your needs may be better suited to Support at Home instead.
