The Restorative Care Pathway exists to help you get there, with the right clinical team behind you and a clear plan from day one.
One thing worth knowing: not every provider can actually deliver this pathway. Avanza can. That is because of the Avanti Health clinical team behind us. That means you won't have to switch providers to access the support you need.
Eligibility is confirmed through My Aged Care. The funding is separate to, and doesn't reduce, any existing support you already receive.
The Return-to-Routine Recovery System
When something changes, a fall, a hospital stay, a gradual loss of confidence, the instinct is to just get some help in place. But help that isn't pointed at something specific tends to drift. Avanza starts by understanding exactly what's changed, sets one clear goal around what getting back means for you, and works toward it deliberately. Here's how that looks in practice.
What changed?
The fall. The hospital stay. The morning when something just felt different. Before anything else, we take time to understand what daily life looked like before, and what specifically has changed. That's what makes a recovery goal mean something, rather than being a box ticked on a form.
What does "back" mean for you?
Not "improve mobility." Something you'd actually know you'd reached: showering safely without help. Making breakfast. Walking to the letterbox. Getting back to the group you loved. When the goal is specific, the whole program has something to aim at, and you know when you've got there.
Rebuild in the real routine.
Avanza brings occupational therapy, exercise physiology, speech pathology and care planning to you, not a generic checklist of services, but the right people for your specific goal, working in your home, around your routine, in the real space where recovery has to happen.
Include the people around you.
Recovery doesn't only happen during the clinical sessions. The people who love your parent, or who live with you, play a real role. Avanza gives them a simple, practical guide covering what to encourage, what not to take over, and when to call us, so everyone is pulling in the same direction.

The pathway needs a qualified clinical care partner and a real allied health team. Most home care coordination providers don't have that, so if you chose one and then needed restorative care, you'd be starting over with someone new. Common questions
The Restorative Care Pathway is a short-term, intensive program within Support at Home, focused entirely on regaining function, not on managing ongoing care needs. It replaced the Short-Term Restorative Care (STRC) program in November 2025. Importantly, it's funded separately to your ongoing Support at Home package: accessing it doesn't reduce your regular funding.
Each episode provides approximately $6,000 in funding for up to 16 weeks of intensive support. Up to two episodes per year are available, at least 90 days apart. Funding is approved as part of a support plan review through My Aged Care, your care planner can help arrange this.
Yes, participants can access the Restorative Care Pathway at the same time as their ongoing Support at Home services. The two funding streams operate separately. Your current support continues unchanged while the restorative care episode runs alongside it.
Not all providers are registered to deliver the Restorative Care Pathway, it requires a qualified restorative care partner with allied health qualifications, which most coordination-only providers don't have. If your current provider can't deliver the pathway, you would need to change providers to access restorative care alongside your ongoing support.
